Feedback triage

Review user feedback, transition through the v0.9 triage state machine (aiqrion.feedback.triage.set_status), and export approved items for evals or training candidates.

• Statuses: new → triaged → accepted → (converted_to_eval | converted_to_training_candidate | fixed) → closed
• Exports honour the v0.7 PII-redaction rules; rows with PII can never enter training candidates
• Issue kinds: response_feedback, hallucination, citation, coding, dispatch, ui_bug, safety_concern, performance, dataset, model_comparison, feature_request
